Issuance of Death Certificates
Ndu Council issues death certificates for individuals who were:
- Residents of the municipality
- Born within the municipality
- Died within the municipality
- Buried within the municipality
Process for Obtaining a Death Certificate:
- Within 90 Days of Death:
- Required Documents:
- National identity card of the deceased
- National identity cards of a declarant and two witnesses (close family relatives of the deceased)
- Required Documents:
- After 90 Days of Death:
- Required Documents:
- National identity card of the deceased
- Two photocopies of identity cards of two witnesses
- A declaratory judgment from a court of law
- Required Documents:
- If the deceased’s national identity card is unavailable, a handwritten application addressed to the Mayor, accompanied by a sum for municipal stamp, is required.
